Practical information for creating questionnaires and application forms on university online interfaces

Via the Corvinus website, it is possible to create online questionnaires or application forms (eg program registration) as part of events, trainings involving University staff or students. IT security resp. due to the uniform data management rules, only the so-called data can be requested via so-called ninja form (e.g. Google Form cannot be used).

1. Questionnaire/blanks

If you want to publish a questionnaire / blank on the university interfaces, fill in the fields to be filled in as a Word document and upload them via the content form. Where there are mandatory fields, clearly indicate them, as well as the explanatory texts that may be included for each field. Please also indicate the type of answer you would like (checkbox, free-text, drop-down menu, etc.). The document also includes the answer options in each case, regardless of the type of question.

2. Data Processing Information

If the questionnaire / form processes personal data, it is necessary to attach a GDPR statement (data management information) to the application. The data management information can be requested from Dr. Marica Sárközi-Kerezsi, Data Protection Officer, at the e-mail address marica.kerezsi@uni-corvinus.hu. Please indicate in the e-mail the subject and purpose of the data processing and the scope of the personal data processed.
